Rural Community Health Improvement Partnership (R-CHIP) Initiative


With funding from the Center for Disease Control and the Maine Department of Health and Human Services, local partnerships covering Kennebec, Somerset, Aroostook, Hancock, and Washington counties were formed and include representation from health care, public health, social service, and other community-based organizations. Partnerships seek to build sustainable service coordination and integration infrastructure to improve residents' health outcomes through addressing individual health-related social needs such as housing instability, lack of transportation, and financial strain.
